We've had a test download recently where the file downloaded with the name "1413817_26M38509GV870573W", without any extension even. It worked fine once the .pdf extension was added, but it should have downloaded as "la_cucina_di_antonella_cookbook,_english.pdf". Any idea what could have caused this?

The gibberish filename without an extension does seem like just a one-time freak occurrence. I'm guessing some transient glitch at Amazon S3 (which we use for long-term download storage and delivery) momentarily failed to associate the proper filename with the file, so it fell back on downloading with the underlying Object ID for that file at Amazon's end.
I issued myself a test link for your product and was unable to reproduce that problem in any browser I tested. If you test your link again now and the problem still happens for you, please reply to tell us what OS (Windows or Mac) and browser app you're using when the problem occurs.
