We have a rule that adds our shipping amount to all items. This shows up properly in the cart but when you go to pay with Paypal it only shows the item amount NOT the shipping. This is a huge issue for us. Any ideas on how we can fix this?

PayPal will show the shipping amount during the course of checkout once the buyer selects a shipping address, before they approve making an actual payment. They do not show the shipping amount at first, just in case the shipping destination given in the cart differs from the actual shipping address which may affect the calculated shipping fee. This is part of the automatic shipping recalculation feature we introduced for PayPal checkouts a few months ago:
